Click on the google form link FIRST and fill out entirely!

After you have submitted your form, click on the Paypal link.

If you have an existing Pay pal account, you will need to login. If you do not have a Pay Pal account, you will need to create one. Follow the steps on Pay Pal to create an account and submit payment.
